Who can request a record
A record is released only to the person it relates to. To apply for somebody else, provide a copy of your own government issued identification and a letter from that person authorising you.
What you will need
- Your full name, including any former names
- Your date of birth
- Your passport number or numbers covering the period requested
- Photo identification
- The period you need the record to cover
- If applying for someone else: your identification, and their written authority
